Hello @kisak-valve; I've tested, Phasmophobia (739630), Disfigure (2083160), CatNab (1881800), Ghost Exorcism INC (1618540), all with the same issue.
Also, after downloading some additional games for testing, Risk of Rain 2 (632360), and ULTRAKILL(1229490) are two Unity games I've found that this issue does not occur.

Any game that uses unity, V-sync is completely unusable. It seems to pick a random arbitrary number to cap at anywhere from 125-165 depending on the unity game, rather than my monitors refresh rate of 240; As well as the performance being AWFUL with V-sync enabled (Games that will run at 350-400 fps, will struggle to stay above 100 fps with V-sync enabled).
This was not an issue with this game (Ghost Exile) last Halloween, as I was able to use V-sync and play at 240 fps smooth.
This becomes in a larger issue in games with framerate caps of 60 that cannot be defeated easily without the use of V-sync, or games with no in game framerate cap running at 400 - 600 fps without the use of V-sync.
This issue does not occur in non-unity games, CS:GO native V-sync works fine, Ready or Not (UE4) V-sync works fine, and a handful of other games of which I do not know which engine they run on.
Reproducible in any unity game.
