Symptoms

Game runs on high core count systems, but seems to run significantly better if you limit the cores.

Reproduction

WINE_CPU_TOPOLOGY=4:0,1,2,3 %command%

The game also suffers quite badly from microstuttering, but this is widely reported in Windows for the same game, and seems to generally be an issue with POEII itself (slash unity).

Sort of, but not completely, related to: https://github.com/ValveSoftware/Proton/issues/5927#

I say related, because the game "runs" but limiting the cores massively helps, so it's different to the linked problem.
