Still issue?
The last time I played (a couple weeks ago), the jitter was still there. However, it seems that the jitter goes away as the game caches models and textures. I think this is related to the slow loading of each level. Loading the assets before the game starts is very slow, and loading un-seen assets is also slow, to the point where it actually pauses gameplay.
The problem I was experiencing was, when I first started playing a level, and a guy came around a corner, my game would pause, and I would die. Then I'd go, "Fuck this!", and ragequit. After playing for a minute or so, the game would stop lagging when I'd see enemies, and would be decently playable.
Issue transferred to ValveSoftware/Source-1-Games#27.

I'm running TF2 on Arch Linux under XFCE4 (compositing disabled) and the latest NVidia drivers (310.19).
TF2 on Windows (with highest video quality settings) runs with a consistent 300fps.
TF2 on Linux (with the lowest video quality settings) runs between 10fps and 300fps.
The FPS seems to correlate to the complexity of the scene. It's very frustrating because whenever I see an enemy (and especially if they're close), the game locks up for a second. It's really hard to kill anyone! :)
Is there any diagnostic information I can give to help sort this out?
