What are your OS details. What is insanely high. Is the server you are connecting to updated. What happens if you run a local server. Have you checked that you have set cl_updaterate, fps_max and rate correctly for your environment.
I use Windows 64-bit.
The ping is relative but can goes as high as 500.
Now that I've tested on some servers it might just be one server. However, this is still an issue since I'm an admin on that server.
And since the server is running AmxModX with some plugins then I'm guessing this might be the issue.
Okay, well check your server out and fix it. Make sure you have updated it and also updated any plugins on it.

When I play on servers, doesn't matter which one it is I get insanely high ping.
And if I have VSync enabled my game becomes hacky.
