During mission Riding shotgun, Alex should activate panel like in this video http://www.youtube.com/watch?v=wXj5uKMua0Q at 5 minute mark

Unfortunatelly she doesn't, so the player can't progress in game.

I have also had this issue , Alyx does not activate and you end up stuck in the train wreckage at the start of the game in Episode 2, I have also had it on Cloud save games in that she refuses to shoot enemys in Episode one - the fix was to restart the chapter.
