Not sure how to reproduce, but often while crouching the player can randomly get stuck in that position. I was just in Office complex near the large Office area in the room where that alien thing eats two scientists, and exploring near the file cabinet. Got stuck in the crouch position. Had to let go of the CTRL key and press it again to get out of the crouched position.

It’s likely moving between maps while crouching caused it, If you can find the spot again, you can open the games console, and type status and it will give map name and coords of the player.
