Pretty much like https://github.com/ValveSoftware/steam-for-linux/issues/281, but the only different thing here is stuttering sound.
This happens for me on machines which don't have much power. On my notebook for example (Core i3, 2.1 GHz), I see a similar behavior. It seems to go away after 5-10 minutes for me so I assume TF2 is loading sounds/materials just-in-time. Since loading takes longer on Linux in general, this might be related and result in delays.
No problems on my desktop, though (Core i7, 3.4 GHz).

I doubt that, 'cause there's no I/O activity at a time like this. Moreover, the stuttering sometimes accompanies by Apport reports 'bout GPU lockup.

As mentioned there, the problem goes off with disablin' semaphores as temporary solution.
I have this same problem on the latest NVIDIA proprietary driver. It happens occasionally, usually every couple of minutes and it does appear to slow down/almost stop after playing for a while. This is the only reason why I still use the windows version under wine (you can't freeze for a couple of seconds/minutes during competitive play).
Also, when this happens the entire X server is completely locked up, nothing updates and even my mouse won't move (until the freeze ends). Sometime the freeze doesn't appear to end (>5min) and I have to poweroff my computer by holding the power button.
