I think I also have this issue, mouse buttons 4 and 5 can be bound within the steam client. But when a game launches the cease to function entirely until you exit the game. I have a Logitech G5 mouse.
For what it's worth, standardized back and forward buttons on a mouse are buttons 8 and 9. The source 1 engine is known to only support 5 mouse buttons -- https://github.com/ValveSoftware/Source-1-Games/issues/68.

Using the side mouse buttons(Mouse button 4 and 5, also doesn't work with Left mouse and right mouse buttons) as the Push-to-Talk key for steam voice chat continuously transmits voice even when the button is not held, basically it works as though if I wasn't using PTT at all and had set the settings so that my voice be transmitted all the time.
Although if I use any key(Caps Lock, A, 1. etc etc) on my keyboard push-to-talk works properly. Mouse buttons(4 and 5 also Left and Right) for PTT works as intended in-game(Dota2 and CS:GO), also works properly in other apps like discord and mumble.

@kisak-valve I think this issue is different than mine, this issues says that they cant bind PTT to mouse keys whereas I can, it's just that it doesn't work, unless I am misunderstanding their issue
Unless I missed something, the issue being tracked here is mouse bindings for steam voice is unreliable. Mouse buttons 1-5 can be bound, but most likely are intercepted by games before steam can get the input. Higher mouse buttons can not be bound.
I see, thank you. Hope this gets fixed soon. Currently have to use caps as the PTT key; people mistake me for being angry a lot xD
I have recently hit this bug too. Almost year has gone without it been fixed.
Well, what I find really strange is that in "push to talk" you need to press the key in order to talk.
However in this situation the key is pressed by default so we "talk" all the time. This cannot be caused by game intercepting input events.
I find it more likely that steam silently disables "push to talk" if mouse button is used, but the change is only internal, it is not saved or displayed. It might have been added as "quick workaround" for some bug and forgotten.
I was able to fix this by using Logitech's software to remap my mouse button to a keystroke. Then I assigned that keystroke to my PTT in Discord. Hope this helps!
