More details please: What distro are you on? Window Manager? What mouse are you using?
Can you use xev and see if the events are showing up there?
And is anyone else having this issue?
Sorry about the vagueness. The distro is Gentoo, and the unreliable behaviour happened in E17.
The mouse is an optical USB mouse by SteelSeries, the Kinzu V2.
After having tried this out in fluxbox, where no such awfulness happens, I'm lead to think this is an issue with E17 (I had similar problems with Half-Life as well, so there's that.) I haven't gotten anyone to repro this on other WMs/DEs.

I've had this issue, too. When I set it to Raw Input, I had several moments where my minigun would stop firing and spin down or my medigun would stop healing my teammate despite them still being in range, all while my mouse buttons are still held down. It only happened when Raw Input was turned on.

Raw mouse input is unreliable and behaves erratically in TF2. Not all mouse presses (like left click, scroll wheel, etc.) are correctly registered, so I have to
m_rawinput 0. This though isn't a very nice workaround.
