This is most likely a driver bug in AMDs Vulkan driver.
The fact that your game is modded and that it takes a random amount of time to reproduce it, makes it impossible to debug this either way without an apitrace.
Installing any mod that adds or removes lines from an .ide/.wpl file mid playthrough will cause save corruption and crashes like this to happen. Only thing you can do is make a new game, the new save won't have this problem.
I've made a new save in TLaD Story, and it still happens nonetheless (exclusively with DXVK installed - playing with native D3D9 renderer causes no such crashes)
Currently experiencing the exact same thing but with a 9070 XT GPU.... the game just freezes with audio still playing, and I have to manually end task from TM. AMD needs to fix their shit. DX9's performance in this game is atrocious, DXVK is almost a necessity.
We recently fixed a crash caused by DXVK memory management. Try the latest commit and report back.

No. It isn't supported by their driver

I don't freeze so much as it just crashes at random intervals.
Im currently also experiencing the same issue. I have a 9070 xt. Is there currently a solution other than reverting to DX9?

Seems like the 26.6.1 driver fixed the issue. No crashes on my end so far.

I've played the game for 1.5 hours on the 26.6.1 driver and didn't crash as well. I'll do couple of more tests later just to be sure

During the gameplay of GTA IV's DLCs (The Ballad Of Gay Tony and The Lost And Damned) the game will freeze complete and stop responding, no matter if You are on a mission or not. This always happens after a random amount of playtime - sometimes it lets me play for just 15-20 minutes, and sometimes for almost an hour. There doesn't seem to be a specific action triggering that - so far it has happened during driving, during walking, right as I pressed "Look Behind" button in car, and after taking a Steam screenshot.
I've tried reverting to the 2023-2024 DXVK versions (because back then said issue was not happening. Though, I've had an RX6600 at that moment), but it did not help
